A Peaceful Journey Hospice is growing our compassionate, patient-centered team and is seeking two Hospice Chaplains / Spiritual Care Providers to serve patients and families throughout the Denver Metro area.

The Hospice Chaplain / Spiritual Care Provider is an integral member of the Hospice Interdisciplinary Group (IDG) and provides individualized spiritual and emotional support to hospice patients and their families.
This is a wonderful opportunity for a chaplain, pastoral counselor, spiritual care provider, or other qualified spiritual-care professional who wants the flexibility of part-time or per-diem work while making a meaningful difference in the lives of patients and families.
You will serve patients primarily in their homes and residential care settings throughout your assigned geographic area.
Provide compassionate, individualized spiritual and emotional care to hospice patients and families.
Complete spiritual assessments and identify patient/family spiritual, religious, existential, and emotional needs.
Develop and implement spiritual-care interventions consistent with the patient's wishes, beliefs, values, culture, and individualized Plan of Care.
Provide supportive presence during times of fear, uncertainty, grief, loss, transition, and end-of-life decision-making.
Offer appropriate prayer, meditation, ritual, scripture, sacraments, music, life review, legacy work, or other spiritual interventions when desired by the patient/family.
Support patients who identify as religious, spiritual, agnostic, atheist, or otherwise non-religious with equal respect and dignity.
Help patients explore meaning, purpose, reconciliation, forgiveness, hope, peace, unfinished business, and legacy when appropriate.
Provide emotional and spiritual support to family members and caregivers experiencing anticipatory grief and loss.
Facilitate connections with the patient's preferred clergy, faith community, pastoral counselor, or other spiritual support when requested.
Participate in the hospice Interdisciplinary Group (IDG) and collaborate closely with nurses, social workers, physicians, aides, bereavement staff, and other members of the care team.
Contribute to individualized Plans of Care and communicate changes in spiritual or emotional needs to the IDG.
Document assessments, visits, interventions, patient/family response, and follow-up recommendations accurately and timely in the electronic medical record.
Participate in IDG meetings, care coordination, quality improvement activities, and hospice education as appropriate.
Provide support surrounding the dying process and, when assigned, participate in appropriate bereavement follow-up.
Maintain professional boundaries, confidentiality, HIPAA compliance, and respect for patient autonomy.
Provide care in accordance with hospice philosophy, Medicare requirements, Colorado requirements, and A Peaceful Journey Hospice policies and procedures.
We are looking for someone who brings both professional spiritual-care skills and genuine human presence.
Education, training, experience, or professional credentials appropriate to the practice of spiritual/pastoral care.
Demonstrated understanding of hospice philosophy, end-of-life care, grief, loss, and family dynamics.
Ability to provide spiritual care that is patient-centered and respectful of diverse religious, spiritual, cultural, and non-religious beliefs.
Strong listening, communication, interpersonal, and documentation skills.
Ability to work independently while functioning effectively as part of an interdisciplinary team.
Ability to maintain professional boundaries and confidentiality.
Ability to meet applicable federal, state, and local requirements for the role.
Successful completion of required background screening and hospice onboarding requirements.
Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.
Master of Divinity (MDiv), Master of Arts in Theology, Pastoral Studies, Spiritual Care, or related graduate degree.
Clinical Pastoral Education (CPE) experience or equivalent supervised clinical spiritual-care training.
Board certification or professional certification in chaplaincy/spiritual care through a recognized professional organization.
Ordination, commissioning, endorsement, or recognition by an established faith tradition, when applicable.
Previous hospice, palliative care, hospital, skilled nursing, behavioral health, or healthcare chaplaincy experience.
Experience providing spiritual care to patients and families facing serious illness, death, dying, and bereavement.
Experience working within an interdisciplinary healthcare team.
Knowledge of grief, loss, trauma-informed care, cultural humility, and end-of-life communication.
Familiarity with electronic medical records and clinical documentation.
Hospice experience is highly valued, but we are willing to consider the right spiritual-care professional with strong transferable experience and a genuine calling to hospice care.
Spiritual care is an important component of the Medicare hospice benefit. CMS requires hospice programs to provide spiritual counseling, including assessment of the spiritual needs of patients and families, spiritual counseling consistent with their beliefs and wishes, and reasonable efforts to facilitate visits from local clergy, pastoral counselors, or other appropriate spiritual supports.
The hospice interdisciplinary approach is designed to address the physical, medical, psychosocial, emotional, and spiritual needs of patients and families facing terminal illness and bereavement.
A Peaceful Journey Hospice maintains personnel qualifications and screening consistent with applicable Medicare and Colorado requirements. CMS requires criminal background checks for hospice employees with direct patient contact or access to patient records, and applicable qualifications must remain current.
