We are seeking a compassionate and experienced Certified Chaplain to serve as our Hospice Chaplain and Bereavement Coordinator. This meaningful hybrid role combines direct spiritual care for hospice patients and families with leadership and coordination of the agency’s bereavement program.


The ideal candidate brings a calm, empathetic presence; respects diverse beliefs and cultural traditions; and understands the emotional and spiritual needs that often accompany serious illness, death, and grief.


As an essential member of the interdisciplinary team, you will help ensure that patients, families, and caregivers receive individualized spiritual and bereavement support consistent with their values, preferences, Plan of Care, and applicable hospice standards.


Position Overview
The Hospice Chaplain/Bereavement Coordinator provides spiritual care, pastoral counseling, and grief support to hospice patients, families, and caregivers.


The position also oversees the agency’s bereavement program, including risk assessments, individualized bereavement plans, grief-support services, memorial activities, and post-death follow-up for a minimum of 13 months.


This is a hybrid position that includes:
  • In-person visits to patients and families
  • Travel to private homes, assisted living communities, nursing facilities, and hospitals
  • Remote or office-based documentation, care coordination, and bereavement-program responsibilities
  • Participation in interdisciplinary team meetings
  • Occasional evening or weekend availability for memorial services or family support
Key Responsibilities


Spiritual Care and Chaplain Services
  • Conduct initial and ongoing spiritual assessments as part of the comprehensive hospice assessment.
  • Identify each patient’s and family’s spiritual needs, beliefs, sources of meaning, concerns, values, and preferences.
  • Develop, implement, and update individualized spiritual Plans of Care.
  • Provide pastoral counseling, spiritual guidance, compassionate presence, and emotional support to patients and families.
  • Facilitate prayer, sacraments, rituals, or other spiritual practices when requested and appropriate.
  • Support patients and families experiencing spiritual distress, fear, anticipatory grief, loss of meaning, conflict, or concerns related to death and dying.
  • Respect and support diverse religious, cultural, spiritual, and nonreligious beliefs without imposing personal beliefs or practices.
  • Coordinate with community clergy, faith leaders, congregations, and other spiritual resources when requested by the patient or family.
  • Communicate significant spiritual concerns or changes to the interdisciplinary team.
  • Participate actively in interdisciplinary team meetings and contribute meaningful spiritual-care updates.
  • Recommend updates to the patient’s Plan of Care based on changing spiritual or emotional needs.
  • Provide hospice staff with education and support concerning spiritual care, grief, loss, and end-of-life needs.
  • Complete all spiritual-care documentation accurately and within agency-required timeframes.
Bereavement Program Coordination
  • Develop, implement, coordinate, and maintain the agency’s comprehensive bereavement program.
  • Help ensure that bereavement services follow applicable federal and state hospice requirements, agency policies, and professional standards.
  • Conduct bereavement risk assessments for patients’ families and caregivers.
  • Identify individuals who may be at increased risk for complicated grief or who may require additional support.
  • Develop and maintain individualized bereavement Plans of Care.
  • Provide grief counseling, education, resources, and support to families and caregivers.
  • Coordinate post-death bereavement follow-up services for a minimum of 13 months.
  • Maintain contact with bereaved individuals through telephone calls, mailings, visits, electronic communication, support groups, and other appropriate methods.
  • Coordinate or facilitate grief-support groups, remembrance activities, and memorial services.
  • Provide community education regarding grief, loss, bereavement, and end-of-life support when appropriate.
  • Refer individuals to community mental health professionals, grief specialists, clergy, or other resources when needs exceed the scope of hospice bereavement services.
  • Maintain accurate and timely documentation of all bereavement assessments, contacts, interventions, referrals, and outcomes.
  • Monitor and evaluate the effectiveness and quality of the agency’s bereavement program.
  • Identify opportunities to strengthen bereavement services and family support.
Interdisciplinary Collaboration
  • Collaborate with physicians, RN Case Managers, social workers, hospice aides, clinical leadership, facility staff, volunteers, and other members of the interdisciplinary team.
  • Promote continuity of care by communicating spiritual, emotional, cultural, and bereavement needs clearly and promptly.
  • Participate in care planning, team conferences, and quality-improvement activities as assigned.
  • Maintain appropriate professional boundaries while providing compassionate support.
  • Protect patient and family confidentiality and comply with HIPAA and agency policies.
Required Qualifications
  • Current certification as a Chaplain.
  • Minimum of one year of pastoral care or counseling experience.
  • Strong understanding of grief, bereavement, end-of-life dynamics, and spiritual distress.
  • Compassionate, empathetic, and nonjudgmental approach to patients and families.
  • Demonstrated cultural, religious, and spiritual sensitivity.
  • Ability to support individuals from diverse faith traditions, cultures, backgrounds, and belief systems.
  • Excellent verbal, written, interpersonal, and documentation skills.
  • Ability to work independently, organize multiple responsibilities, and meet required deadlines.
  • Ability to work effectively as part of an interdisciplinary hospice team.
  • Ability to travel frequently to patients’ places of residence and healthcare facilities.
  • Ability to successfully complete required background checks, certification verification, and onboarding requirements.
  • Commitment to maintaining confidentiality and complying with HIPAA and agency policies.
Preferred Qualifications
  • Pastoral-care or counseling experience in hospice, palliative care, a hospital, or another healthcare setting.
  • Experience providing grief counseling or bereavement support.
  • Experience developing or coordinating a hospice bereavement program.
  • Experience facilitating grief-support groups, remembrance events, or memorial services.
  • Familiarity with hospice documentation and electronic medical record systems.
Working Conditions
  • Frequent local travel to patient homes, assisted living communities, nursing facilities, hospitals, and other care settings.
  • Regular interaction with patients and families experiencing terminal illness, death, grief, and emotionally intense circumstances.
  • Hybrid responsibilities involving field visits and remote or office-based program coordination.
  • Occasional evening or weekend availability for memorial services, support groups, or urgent family needs.
  • Ability to manage emotional demands while maintaining professionalism, compassion, and appropriate boundaries.
