    Hospice Bereavement Coordinator - HomeCare - (26159447)

    Description

    To provide supportive services to help meet the needs of the terminally ill Hospice patient and family as needed. To provide assistance and understanding to the family in time of bereavement. To work as a member of the Hospice team in providing Hospice care. Responsible for developing, implementing and supervising the bereavement program and for the delivery of related services.

    Manage the bereavement services program utilizing professional staff and volunteers. Oversee adequacy and appropriateness of bereavement programs for patient and family members.

    Develop new bereavement programs and services which will include educational programs and materials for Patients/families, and the community regarding loss, grief and coping with bereavement.

    Participates in Hospice educational programs.

    Design materials for distribution to families eligible for and/or receiving Hospice bereavement services.

    Plans, implements and supervises bereavement group events.

    Oversees bereavement follow-up by patient care staff and volunteers.

    Attends and participates in IDG conference relative to bereavement risk assessment and follow up.

    Qualifications

    Minimum Requirements:

    Bachelor''s degree with training in pastoral care

    1 year experience in pastoral care, including experience in counseling the dying and grieving

    Preferred Requirements:

    Graduate from an accredited institution of social work or theology

    More than1 year of supporting the dying and grieving
