    Hospice & Palliative Care Physician

    Full-Time | Monday-Friday, 8:30 a.m. to 5:00 p.m.On-Call Rotation | Weekends & Holidays as Needed

    We are seeking a compassionate and collaborative Hospice & Palliative Care Physician to join our interdisciplinary medical team. This position offers the opportunity to provide direct patient care across multiple settings while helping shape the future of hospice and palliative medicine within one of North Carolina's most respected nonprofit organizations. Transitions LifeCare pioneered one of the state's first palliative care programs and has been serving the community for more than four decades.

      What You'll Do

      Deliver Expert Hospice & Palliative Care

      • Provide hospice and palliative care consultations in patients' homes, residential facilities, hospitals, and hospice inpatient settings
      • Manage complex pain and symptom needs utilizing evidence-based practices
      • Develop individualized treatment recommendations aligned with patients' goals and values
      • Support patients and families through serious illness, advanced disease, and end-of-life decision-making
      • Collaborate with attending physicians and care teams to ensure seamless care coordination

      Serve as a Clinical Leader

      • Participate as an active member of interdisciplinary team meetings
      • Provide medical guidance to hospice and palliative care clinicians
      • Support and oversee advanced practice providers
      • Serve as a collaborating physician when appropriate
      • Contribute to clinical quality initiatives and program development

      Shape the Future of Care

      • Educate community physicians, hospital staff, and healthcare partners about hospice and palliative medicine
      • Teach and mentor students, residents, and fellows
      • Participate in development and review of medical policies, procedures, and clinical standards
      • Support quality improvement initiatives, outcomes evaluation, and program growth

      Ensure Exceptional Patient Care

      • Complete timely and compliant medical documentation
      • Participate in hospice certifications and recertifications as required
      • Assist in maintaining regulatory, accreditation, and compliance standards
      • Participate in physician on-call coverage to ensure 24/7 physician availability

      Required Qualifications

      • MD or DO from an accredited medical school
      • Completion of residency training in a specialty appropriate for Hospice and Palliative Medicine
      • Board certification in an applicable specialty
      • Current North Carolina physician license
      • Current DEA registration
      • Current CPR certification
      • Eligibility for credentialing with Transitions LifeCare and affiliated hospitals
      • Ability to maintain required CME standards
      • Valid North Carolina driver's license
      • Reliable transportation and automobile insurance

      Preferred Qualifications

      • Hospice and Palliative Medicine board certification
      • Prior hospice and/or palliative care experience
      • Experience supervising advanced practice providers
      • Experience working within interdisciplinary care teams
      • Interest in physician education, mentorship, and community outreach
