Responsibilities:
- Provide management and delivery of patient care plans
- Support patients in Glen Carbon and surrounding areas
- Reward the accomplishment of key achievements during the first year
Qualifications:
Required
- Associate degree or higher
- Hospice (2+ years)
- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart HER
- Valid Registered Nurse (RN) in the state of Illinois (IL)
- Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)? (yes)
- Are you okay with the required travel for this position? (yes)
- Have you applied to or worked for Residential anytime in the last 6 months? (no)
Preferred
- Please provide your RN License Number.
Schedule and Shift Details
It is 7 evenings in a row from 5 PM to 8 AM Mon to Sun after hours on call and making visits if needed in the Springfield IL area. Then would be off 7 days in a row.
