About the Role:
The Hospice Admissions Coordinator in Roswell plays a critical role in facilitating the admission process for patients entering hospice care, ensuring a compassionate and efficient transition during a sensitive time. This position serves as the primary point of contact for patients, families, and healthcare providers, coordinating timely admissions while maintaining compliance with regulatory standards. The coordinator is responsible for gathering and verifying patient information, assessing eligibility, and collaborating with interdisciplinary teams to develop individualized care plans.
