Responsibilities of a Nurse Case Manager include:

• Manage a case load of home care patients.
• Assess, plan, implement, coordinate, and evaluate care for each patient.
• Appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
• Supervise Home Care staff as necessary.

Qualifications for a Nurse Case Manager include:

• ASN required, BSN or MSN preferred.
• Must possess an active RN license in both VT and NH.
• Weekend and on-call rotation required.
