    We’re Hiring: Transitional Aide

    We’re looking for a caring Transitional Aide who is flexible and willing to work overnight and various weekend hours. This role is perfect for someone who wants to make a difference and can adapt to changing client needs


    QUALIFICATIONS:

    Must meet Home Health Aide (HHA) /CNA requirements

    Home Health / or CNA License 

    Ability to pass background check and drug screening

    Must be flexible with scheduling, including overnights and weekend availability is required with this position

    Compassionate, reliable, and dependable

    Previous home health or caregiving experience preferred

    Job duties:

    Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility

    Provide companionship and emotional support to clients

    Assist with meal preparation, feeding, and hydration as needed

    Perform light housekeeping related to client care (laundry, dishes, tidying client areas)

    Assist with safe transfers and ambulation, using proper techniques and equipment

    Observe, report, and document changes in client condition to the supervising nurse or office staff

    Follow individualized care plans and agency policies at all times

    Provide support during transitions of care (hospital to home, facility to home, short-term care needs)

    Maintain a safe, clean, and comfortable environment for clients

    Respect client privacy, dignity, and confidentiality

    Communicate effectively with clients, families, and the care team
