Wage Range: $41.12 - $61.68 hourly including $2.00 an hour lead stipend; depending on number of years of transferable experience and internal equity

Home Health Program Overview:   

RiverStone Health Home Health Services provides medical treatment for an illness or injury, to help patients recover, regain independence, and become as self-sufficient as possible. Home health care helps patients manage chronic conditions like heart disease, diabetes, pain management or chronic obstructive pulmonary disease (COPD) and provides personal care services.  Home Health Services may be requested by physicians, the patient or family member, or a hospital discharge planner.  Through consultation with the patient’s healthcare provider, Home Health Services develop a plan of care to meet the patient’s individual healthcare needs.  The team includes nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and certified home health aides.  

 Job Summary:  

 The Therapy Supervisor is responsible for the delivery of quality patient care through the therapy process of assessment, diagnosing, planning, implementation, and evaluation. In tandem with the clinical manager and clinical therapy staff, the therapy supervisor helps to oversee patient care, PT/OT/ST staff, clinical documentation (OASIS), referral staff, and customer satisfaction with home health patient care services. The incumbent brings content expertise and knowledge surrounding the program elements, deliverables, and metrics. The work environment is typically fast paced.  Work is performed to ensure efficiency of program delivery, incorporating integrity and innovation to ensure a high level of customer service. 

Essential Functions/Major Duties and Responsibilities:  

Physical Therapist75%    

  • Provide skilled, comprehensive physical therapy services in a home‑health setting. You will work both independently and collaboratively to deliver direct care, education, and coordination to patients and their families, all within your licensed scope of practice. 
  • Independently administer and monitor therapeutic interventions in compliance with physician orders and accepted PT practice standards. 
  • Conduct evaluations and interpret symptoms and treatment responses, collaborating with interdisciplinary healthcare providers to develop and adjust individualized plans of care. 
  • Deliver education and training to patients and caregivers on managing impairments, using adaptive equipment, and promoting functional independence. 
  • Provide skilled interventions tailored to physical, cognitive, communicative, and functional deficits, including swallowing/dysphagia management for SLP clients. 
  • Initiate and conduct admissions and home visits, establish priorities and goals, and coordinate care based on patient needs. 
  • Consult and coordinate with physicians, nurses, therapists, home health aides, and other care providers throughout the patient’s care continuum. 
  • Communicate changes in patient condition promptly to the care team, physician, and family to ensure continuity and safety. 
  • Participate in case conferences—whether in-person, telephone, or electronic—to support collaborative treatment planning. 
  • Document evaluations, treatment plans, interventions, patient responses, progress, and discharge in the electronic medical record, adhering to documentation standards and completing charting in a timely manner. 
  • Demonstrate knowledge of the Outcome and Assessment Information Set (OASIS‑E), include quality and reimbursement metrics in documentation and care delivery. 
  • Understand and adhere to Medicare/Medicaid guidelines, HIPAA, state regulations, and accreditation standards relevant to home health services. 
  • Maintain universal precautions and follow organizational safety protocols at all times. 
  • Participate in performance improvement initiatives aimed at achieving positive patient outcomes. 
  • Receive and address patient or family concerns in a professional, timely manner. 
  • Pursue ongoing professional growth through orientation, continuing education, and departmental training. 
  • Demonstrate competencies—interpersonal, cognitive, and technical—that align with the organization’s mission and standards. 
  • Participate in a weekly rotating on-call schedule with the Clinical Manager, providing after-hours, weekend, and holiday support. 
  • Carry program-issued laptop and phone to manage patient care operations while on call. 
  • Fulfill other home health coverage assignments as directed.                     

Therapy Supervisor20% 

  • Coordinates therapy staff by assisting with scheduling. 
  • Provides therapy-related clinical input to determine appropriate referrals for acceptance and assists in identifying the clinical information needed for admission, medication entry, and intakes. 
  • Assists therapy staff in decision making, care coordination, and time management in cases with complex needs  
  • Conducts regular conferences with all home health team members to promote information exchange, determine care assignments, and to promote integration of care plans 
  • Assures that all therapy staff members deliver respectful care consistent with the patient/family's home, cultural, and environmental circumstances. 
  • Assesses the therapy team’s ongoing patient care through care conferencing and review of documentation to determine educational and care needs.  
  • Ensures program and agency policies and procedures are being followed by therapy team 
  • Oversees and ensures accurate and timely documentation with appropriate utilization of medical records systems by the therapy team 
  • Assists therapy team with ensuring quality care and patient/client satisfaction 
  • Identifies and supports quality improvement initiatives for the therapy team. 
  • Attends meetings, conferences, workshops and training sessions, and reviews publications and audio-visual materials to become and remain current on principles, practices and new developments related to the therapy and home health field.  
  • Develops and maintains trusting relationships with patients, clients and RiverStone Health staff, and community referral partners.  
  • Responds to questions, comments and concerns in a courteous, effective, and timely manner.  
  • Promotes teamwork and collaboration among staff. 
  • Works closely with other RiverStone Health staff and participates in team meetings to develop plans and ideas for program expansion, delivery and quality improvement initiatives.  
  • Keeps immediate supervisor and designated others accurately informed concerning work progress, including present and potential work problems and suggestions for new or improved ways of addressing such problems 
  • Handles corrective action for therapy staff 
  • Directs the work of therapy staff, establishes therapy work schedules, reviews and approve therapy timesheets. 
  • Provides professional development, training, and support for assigned therapy staff. 
  • Works in collaboration with Human Resources when recruiting, hiring, and onboarding therapy staff; encountering behavior, performance, or reliability issues; and when therapy staff request extended leave, or work accommodations. 
  • Completes regular therapy performance appraisals; handle employee therapy relations incidents in a timely manner. 
  • Keeps current on best therapy supervision practices through research and trainings and ensuring compliance with applicable policies and procedures

Education and Experience:  

Minimum Qualifications 

  • Graduation from accredited PT program. 
  • 3 years experience with rehabilitation and/or disabilities 
  • Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work.  

Preferred Qualifications: 

  • Experience working with an underserved population  

Required Certificates, Licenses, Registrations: 

  • Licensed as a PT in the State of Montana in good standing.

Supervision:  

  • Supervision of therapy staff 
  • 3 FTEs (1 PT, 1 PTA, 2 OTs) 
  • 1-PRN PT 
  • 1- PRN ST 

Physical Demands and Working Conditions:  

  • Required to stand, walk and sit; talk or hear, both in person and by telephone; use hands to finger, handle or feel objects or controls; reach with hands and arms.  
  • Regularly required to stoop, kneel, bend, crouch and lift up to 25 pounds frequently and 50 pounds occasionally; expected to ask for help from a colleague or utilize a lift when assistance is needed.  
  • Specific vision abilities required by this job include close vision, distance vision, depth perception, color vision and the ability to adjust focus. 
  • Ability to travel and use own personal vehicle. Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, other vehicles, etc. Travel occurs in all weather conditions, including extreme heat and cold. 
  • Risk of needlestick injury is possible.   
  • Position is regularly exposed to biological pathogens and contagious diseases where Personal Protective Equipment (PPE) and special precautions must be taken due to reduce exposure and mitigate risk. 
  • Will be exposed to home environments that may include allergens such as smoke, pets, etc.  
  • Create and maintain a safe/secure working environment by adhering to safety, security, and health requirements. Integrate injury, illness, and loss prevention into job activities by attending any necessary training and implementing best practices.

  • Contacts occur both inside and outside RiverStone Health and typically are for the purposes of rendering services to patients to gather information and provide education and instruction.    
  • Communication with others is a critical element of the position because the purpose of the contract may be to negotiate matters involving significant issues of importance. 
  • Communication and coordination with others is necessary for effective and efficient inter- and intra-agency operations and activities. 

Budget & Resource Management: 

  • This position is responsible for managing resources including time, equipment, and other resources necessary to deliver services. Decisions made by the position directly affect the efficiency and cost of services.
