Speech Therapist Job Description
The Speech Therapist will provide skilled speech-language pathology services to patients in a home health setting. The ideal candidate will be organized, clinically confident, and committed to delivering high-quality patient-centered care.
- Provide skilled speech-language pathology services to patients in their homes
- Evaluate patients with speech, language, communication, cognitive, and swallowing disorders
- Develop individualized treatment plans based on patient needs, diagnoses, and functional goals
- Provide therapeutic interventions designed to improve communication, cognition, and swallowing function
- Assess and treat patients with a variety of neurological, medical, and age-related conditions
- Educate patients, family members, and caregivers regarding treatment plans, compensatory strategies, and home exercise programs
- Recommend appropriate communication strategies and adaptive equipment when clinically indicated
- Monitor patient progress and modify treatment plans as needed
- Complete accurate and timely clinical documentation in accordance with home health standards
- Collaborate with physicians, nurses, occupational therapists, physical therapists, social workers, and other members of the interdisciplinary care team
- Participate in care coordination and communicate changes in patient condition as appropriate
- Provide patient and caregiver education to promote safety, independence, and carryover of therapeutic strategies
- Follow all applicable state, federal, and organizational requirements for home health speech therapy
- Maintain a professional and compassionate approach while working independently in patients' homes
Education and Certification Requirements
- Master's degree in Speech-Language Pathology from an accredited program
- Current Idaho Speech-Language Pathology license or eligibility for Idaho licensure
- Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) preferred/required according to applicable licensing standards
- Current ASHA certification preferred
- Current CPR certification preferred
- Valid driver's license and reliable transportation
- Ability to travel to patient homes throughout the assigned service area
- Previous home health speech therapy experience preferred
- Strong clinical assessment, documentation, communication, and organizational skills
Preferred Qualifications
- Experience providing speech therapy in a home health setting
- Experience treating adult and/or geriatric patients
- Knowledge of dysphagia and swallowing disorders
- Experience with cognitive-communication disorders
- Experience with neurological conditions and rehabilitation
- Strong patient and caregiver education skills
- Ability to work independently while maintaining communication with the interdisciplinary care team
- Excellent time management and scheduling skills
- Commitment to patient-centered care and functional outcomes
