Home Health Registered Nurse (RN) – San Antonio, TX

Weekly Hour Guarantee: 36 hours

Productivity Expectation: 30 points

Coverage Areas: 60 radial miles

Mileage Reimbursement: Yes, reimbursed between visits, from first patient to last patient

On Call: On a rotation

Shift schedule (days, start & end times): M-F 8-430pm

Position Overview

We’re looking for a dedicated Home Health RN to join our San Antonio, TX team. This role is ideal for someone who thrives in autonomous patient care, enjoys meaningful one‑on‑one interactions, and is confident managing a diverse home‑health caseload. 

 

Responsibilities

• Complete comprehensive assessments, initiate care plans, and establish patient baselines.
• Evaluate patient status after hospitalization or interruption in services.
• Conduct timely reassessments to ensure continued eligibility and update care plans.

 

Required Qualifications

• Active RN License in the state of TX
• Strong clinical judgment and ability to work independently in the field
• Reliable transportation
• Home health experience
