Job Details: 

Schedule: 5 days per week | 8-hour shifts
Shift Timings: 8:00 AM – 4:30 PM
Location: Hollsopple, PA
Float / Coverage Area: Somerset County
Job Type: Contract
Contract Length: 13 Weeks (subject to confirmation)
Productivity: 30 points per week
On-Call Requirement: 1–2 days per week and every 4th–5th weekend for weekend on-call coverage
Mileage Reimbursement: Yes – between patient visits
Pay Range - $53 to $56/ hr on W2 
Pay offered is based on experience, expertise, credentialing, and education.  

Duties:

• Conduct comprehensive nursing assessments and provide skilled nursing care to patients in their homes.

• Complete OASIS assessments and maintain accurate, timely clinical documentation.

• Provide wound care, wound VAC management, and PICC line care.

• Perform laboratory draws and other skilled nursing procedures as required.

• Develop, implement, and evaluate individualized patient care plans in coordination with physicians and the interdisciplinary healthcare team.

• Monitor patient progress, identify changes in condition, and communicate updates to the appropriate healthcare providers.

• Utilize Home Care Home Base (HCHB) for clinical documentation and patient care management.

• Meet the weekly productivity expectation of 30 points while maintaining quality patient care. 

Requirements: 

• Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).

• License(s): Active Registered Nurse (RN) License.

• Certification(s): BLS/CPR Certification.

• Years of Experience: Previous Home Health nursing experience required.

• Skills/Specialties/Technology:

  • Home Health nursing experience.

  • OASIS documentation and assessments.

  • Home Care Home Base (HCHB) experience.

  • Wound care and wound VAC management.

  • PICC line care and laboratory draws.

  • Strong patient assessment and clinical documentation skills.
