Lead with purpose. Have room for life.

You know home health. You understand the clinical judgment, coordination, and attention to detail it takes to deliver exceptional care in someone's home. Now you're ready to use that experience to support clinicians, strengthen operations, and help lead a team.

At National Church Residences Home Health, we're looking for an experienced RN to partner with our Clinical Directors across our Central and Southern Ohio agencies. This role functions much like an Assistant Director of Nursing, combining hands-on clinical leadership with staff development, quality improvement, and daily operational support.

You'll have the opportunity to make a meaningful impact-and the flexibility to manage life outside of work.

Why this role stands out

  • Choose the office that works for you. Be based in either Columbus or Chillicothe while supporting both agencies.
  • Work from home one day each week. A hybrid arrangement offers flexibility while keeping you connected to your team.
  • Enjoy flexibility within your workday. Have a personal appointment? We support coordinating your schedule around life's needs while ensuring clinical responsibilities and team coverage are met.
  • Lead alongside other leaders. Partner directly with our Clinical Directors to solve problems, support staff, and improve care.
  • Put your ideas into practice. Help improve workflows, strengthen clinical processes, and make the workday better for the people delivering care.
  • Connect your work to a meaningful mission. Help older adults receive quality care where they live.

How you'll make a difference

Be a trusted clinical resource.

Help field clinicians navigate patient concerns, care planning, changes in condition, and coordination with providers. Support sound clinical decisions and timely follow-through.

Partner with our Clinical Directors.

Help manage daily clinical operations, address emerging needs, and keep priorities moving across both agencies.

Strengthen quality and survey readiness.

Review care plans and clinical documentation, conduct audits, identify opportunities for improvement, and help translate home health requirements into clear, practical guidance for staff.

Develop and support the team.

Assist with onboarding, education, mentoring, supervision, and performance management. Help clinicians build confidence and competence.

Coordinate care and resources.

Support admissions, recertifications, utilization review, and scheduling to align patient needs with appropriate clinical resources.

Keep workflows moving.

Use Homecare Homebase (HCHB) to monitor clinical tasks, documentation, and follow-up. Identify barriers and help implement solutions.

Share in clinical on-call coverage.

Participate in a rotating on-call schedule to support patients and clinicians outside regular business hours.

What you'll bring

  • An active RN license in Ohio.
  • At least two years of home health leadership or clinical operations experience.
  • A strong working understanding of the home health landscape, including Medicare Conditions of Participation, OASIS, care planning, documentation requirements, reimbursement, and regulatory compliance.
  • The ability to turn clinical and regulatory knowledge into practical guidance for others.
  • Strong organization, prioritization, and follow-through, with the ability to manage competing needs across two agencies.
  • A collaborative leadership style and the confidence to coach staff, address concerns, and hold clear expectations.
  • Comfort with Microsoft Office and electronic medical records; HCHB experience is a plus.
  • A valid driver's license and reliable transportation, with travel of up to 25%.
