Well established home care agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include the
Norwood/
Westwood, MA area and surrounding towns.
- Monday-Friday, 40 hours/week
- No weekends or holidays!
- Occasional on-call as needed

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
