HOME HEALTH RN CASE MANAGER – LOCAL CONTRACT

You will manage a caseload of home health patients, completing admissions, recertifications, discharges and care coordination with physicians and families. Join a hospital-affiliated home care agency serving the greater Rochester area.

Pay $55 - $60 p/h (W-2)

Location Rochester, NY 14642 and surrounding Monroe County

Contract 13 weeks, extension possible

Schedule Monday Friday days, field-based (varies by order)

Requirements

• Active New York RN license (New York is not a compact state)
• 2+ years of RN experience, including 1+ year of home health case management
• OASIS proficiency (start of care, recertification, discharge)
• Valid driver's license, reliable vehicle and auto insurance
• Current BLS
