Well established home health agency has a need for an experienced
Home Health RN Case Manager to see patients in homes with coverage territory in and around the
Brockton, MA area.
- Monday-Friday, 40 hours/week
- Full-time, Contract
- No weekends/on-call/holidays required!

The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
