Well established home health agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include the
Attleboro/Norton/Mansfield, MA area.
- Monday-Friday, 40 hours/week
- Weekend rotation
- Highly competitive pay rate plus IRS mileage reimbursement
- 13-week contract
The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.Great benefits include:- Health, Dental, Vision
