A leading healthcare organization is seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols
Assess, plan, implement, and evaluate individualized patient care plans
Oversee and coordinate the activities of the multidisciplinary team involved in patient care
Monitor the delivery of therapeutic services through effective written and verbal communication with all disciplines
Ensure all scheduled home visits are authorized and supported by physician orders
Conduct weekly reviews of patient visits and communicate care plans and anticipated needs to the Patient Care Supervisor and team assistant
Communicate schedule changes, caseload updates, and patient care concerns to the Home Care Supervisor
Maintain accurate and timely documentation of patient assessments and care provided
Educate patients and caregivers to promote health, independence, and positive outcomes
Collaborate with interdisciplinary team members to ensure continuity and quality of care
Graduate of an accredited School of Nursing
Current Registered Nurse (RN) license in the state of practice
Current Basic Life Support (BLS) certification
Valid Florida driver's license or ability to obtain one within 30 days of hire
Associate Degree or Diploma in Nursing required
Bachelor of Science in Nursing (BSN) preferred
Minimum of one year of Registered Nurse experience required
Previous home health experience preferred
Strong communication, organizational, and critical thinking skills
Ability to work independently and effectively manage patient caseloads
Must meet all applicable background screening requirements
