Home Health Registered Nurse (RN)

Altruist Home Health Care, Inc., is seeking a Home Health Care Admissions Nurse with a minimum of 2 years of experience in a home healthcare setting. The candidate must have experience with Oasis Admissions and a current and valid unrestricted Registered Nursing license to work in Texas.  We promote diversity and inclusion among our staff and patient care services as well as competitive pay and flexible schedules.

Primary Functions:

• Oasis Admissions for Home Health Care Patients
• Administer skilled nursing care for clients of all ages in their place of residence including wound care, wound vacs, IV therapy, venipunctures, patient and caregiver education, etc., and be proficient and timely with clinical documentation. 
• Coordinate care with the interdisciplinary team, patient/family, and referring agency
• Assumes the responsibility for coordination of care
• Required to participate in the daily, weekly, and monthly meetings

 Skills: 

• Nursing skills as defined as generally accepted standards of practice.
• Excellent interpersonal skills and ability to communicate effectively.
• Proof of current CPR, negative TB screen, and Hepatitis profile.
• Compassionate about home health care.
• Dependable
• Attentive to detail
• Spanish speaking a plus!

Experience:

• Two (2) years of experience as a Registered Nurse in a home health setting is REQUIRED.
• PreferredHom to have knowledge and experience with various EMR systems including AXXESS.

Education:

• Graduate of an accredited Diploma
• Associate or Baccalaureate School of Nursing

License/Certifications:

• Current state license as a Registered Nurse
• Current and Valid Texas Driver's License

Requirements:

• Must provide proof of current and valid auto insurance.
• Must provide proof of TB Skin Test result.
• Must have an impeccable work history with professional references.
• Must be willing to see patients in the surrounding metropolitan area.
