Position Overview
Seeking an experienced and compassionate Hospice Registered Nurse (RN) to join our team in Flagstaff, Arizona. The RN Case Manager will provide comprehensive hospice care to patients within a 60-mile radius of the agency, managing a caseload of up to 15 patients. This role requires the ability to perform all visit types, including routine patient visits, admissions, and death visits, while delivering exceptional end-of-life care and support to patients and their families.
Schedule & Work Details
- Location: Flagstaff, AZ
- Coverage Area: 60-mile radius from the agency
- Schedule: Monday through Friday, 8:00 AM - 4:30 PM
- On-Call Schedule: 4:30 PM - 8:00 AM Monday through Friday
- Weekend On-Call: Friday 4:30 PM through Monday 8:00 AM (rotating schedule)
- Weekly Hour Guarantee: 40 hours
- Pay Rate:$51.00 - $53.00 per hour
- Mileage Reimbursement: Yes, reimbursed for travel between patient visits
Key Responsibilities
- Manage and coordinate care for a hospice patient caseload of up to 15 patients.
- Complete routine nursing visits, admissions, recertifications, and death visits.
- Assess patient conditions and develop, implement, and update individualized care plans.
- Collaborate with physicians, interdisciplinary team members, patients, and families to ensure quality hospice care.
- Provide patient and family education regarding disease processes, symptom management, medications, and end-of-life care.
- Accurately document all patient care activities within the HCHB (Homecare Homebase) system.
- Participate in the on-call rotation, providing after-hours and weekend patient support as needed.
- Maintain compliance with agency policies, state regulations, and hospice care standards.
Required Qualifications
- Active Arizona Registered Nurse (RN) license or Compact RN License.
- Recent hospice nursing experience.
- Recent Homecare Homebase (HCHB) experience.
- Strong clinical assessment, documentation, and care coordination skills.
- Ability to work independently and manage a patient caseload effectively.
- Valid driver's license and reliable transportation.

- Experience managing a hospice caseload independently.
- Strong communication and interpersonal skills.
- Experience performing admissions, routine visits, and death visits.
