Job Summary

We are seeking a compassionate and experienced Registered Nurse to provide comprehensive home health care services. The RN will manage care plans, administer treatments, and collaborate with multidisciplinary teams to support patients in their homes.

Qualifications

  • Active Connecticut RN license
  • Minimum 2 years of nursing experience (home health preferred)
  • Reliable transportation for patient visits
  • BLS Certification preferred
  • Strong communication, organizational, and critical thinking skills
  • Knowledge of OASIS

What You’ll Do

    • Perform clinical assessments and provide nursing interventions including wound care, medication administration, and procedures
    • Monitor and document changes in patient condition
    • Execute and follow up on physician orders
    • Collaborate with home health aides and other care providers
    • Manage care coordination and ensure quality, patient-centered care

Compensation

Hourly: $65.00 - $75.00 per hour
