    What You’ll Do

    As a Home Health RN, you’ll bring skilled, heartfelt care directly to patients and families. Your days will be filled with purpose as you:

    • Provide one‑on‑one nursing care in the comfort of patients’ homes
    • Complete assessments, develop care plans, and deliver high‑quality clinical care
    • Educate patients and families to support independence and confidence
    • Collaborate with physicians, therapists, and your care team to ensure seamless care
    • Document visits accurately and timely using our user‑friendly EMR
    • Support patients through recovery, chronic condition management, and health maintenance

    This role is perfect for a nurse who enjoys autonomy, meaningful patient time, and the ability to truly see the impact of their care.

    Requirements

    What You'll Bring

    • Active RN license
    • Active BLS certification
    • Active CPR certification
    • Experience with OASIS-based home health documentation
    • Experience with care planning, medication management, and coordination of care
    • Experience working with patients and families, physicians, therapists, and the full care team
    • Comfort with technology and electronic documentation systems
    • Eligible to work in the United States
    • Reliable transportation
    • Home health experience is a plus, but not required — we’re happy to train the right nurse
    • Bachelor of Science in Nursing (BSN) is a plus
    • Experience with Kinnser is a plus
