Overview of Position
The Home Health Quality Program Manager provides strategic oversight of quality and performance improvement efforts to support regulatory compliance, consistent clinical practices, and positive patient outcomes. This position reports to the Executive Director of System Quality Improvement and maintains a dotted-line relationship with the VP of Home Health.
Job Responsibilities:
- Oversee OASIS submission timelines, provide submission backup, monitor regulatory changes, and educate clinical and quality teams on accuracy and compliance.
- Monitor individual and team OASIS performance, quality-of-care scores, and patient outcomes; partner with Directors of Nursing to identify trends and implement improvements.
- Develop, implement, and monitor the BRHH Quality Assessment and Performance Improvement plan, including leading QAPI meetings, tracking performance indicators, and reporting progress to leadership.
- Produce and communicate timely quality, value-based purchasing, Star Ratings, patient satisfaction, and other required reports; serve as backup for monthly patient satisfaction submissions.
- Conduct routine and random clinical chart audits, analyze findings, and collaborate with Medical Records and Clinical Managers to improve documentation accuracy, care quality, and timeliness.
- Facilitate readmission and root-cause reviews and serve as the Home Health liaison to the Corporate Risk Manager for identified risks.
- Develop performance indicators and audit tools to assess compliance with federal and state regulations, OSHA guidelines, and agency policies and procedures.
- Interpret regulatory and policy requirements with leadership and coordinate staff education with Directors of Nursing and the Education Department.
- Serve as the primary liaison for McBee contract administration, including regular meetings, workflow oversight, timely implementation, and identification of improvement opportunities.
- Serve as the quality liaison for preferred-provider facilities, providing patient updates and reports for bimonthly and quarterly meetings.
- Coordinate division-wide performance improvement initiatives by organizing meetings, tracking and trending data, monitoring progress, and recommending operational improvements.
- Promote teamwork, positive morale, sound decision-making, and consistent use of appropriate home care procedures in support of the agency's mission and goals.
Job Qualifications:
- Licensed Registered RN with Home Health experience and experience with OASIS reviews required; BSN with 2 years of OASIS experience preferred
- Three to five years of home health quality, compliance, or performance improvement experience.
- Demonstrated knowledge of CMS Conditions of Participation, QAPI requirements, Home Health Value-Based Purchasing, Star Ratings, and patient satisfaction measures.
- Experience conducting clinical record audits and root-cause analyses.
- Proficiency in analyzing quality data, identifying trends, and presenting actionable findings to clinical and executive leaders.
- Demonstrated ability to lead cross-functional initiatives, educate clinical staff, manage vendor relationships, and communicate regulatory requirements effectively.
- Strong organizational, project management, critical-thinking, and interpersonal skills.
- Proficiency with electronic health records, Microsoft Office applications, and quality reporting systems preferred.
Location: Home Health Corporate Office Jacksonville, FL.
Hours: Monday through Friday; 8am to 5pm. Occasional nights and weekends which can be remote. Work from home may be accommodated on a very limited basis, such as 1 day per week.
