DirectMed also specializes in caring for patients receiving outpatient dialysis, providing specialized clinical training and support to our therapy staff.


Competitive Per-Visit Compensation


We offer the following visit rates:

• $120 – Start of Care (SOC)
  
  
• $95 – Regular Physical Therapy Visit
  
  
• $100 – Recertification
  
  

Additional premium rates may be available for weekend visits.


Qualifications & Requirements

• Active Physical Therapist (PT) license in the State of Illinois.
  
  
• Bilingual in Polish and English.
  
  
• Minimum of one year of physical therapy experience in a hospital, rehabilitation facility, nursing home, or home health setting.
  
  
• Home health experience preferred but not required.
  
  
• Experience with Medicare home health documentation and OASIS assessments preferred.
  
  
• Excellent written and verbal communication skills.
  
  
• Ability to work independently and manage assigned patients.
  
  
• Reliable transportation.
  
  
• Strong organizational and time-management skills.
  
  
• A positive, dependable, can-do attitude.
  
  

Job Responsibilities

• Provide skilled physical therapy services to patients in their homes.
  
  
• Perform comprehensive physical therapy evaluations and assessments.
  
  
• Complete Start of Care and Recertification assessments when assigned and permitted under applicable home health requirements.
  
  
• Develop and implement individualized physical therapy treatment plans.
  
  
• Provide gait training, balance training, strengthening, therapeutic exercises, and functional mobility training.
  
  
• Assist patients with improving independence, mobility, safety, and overall functional abilities.
  
  
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
  
  
• Coordinate care with physicians, nurses, occupational therapists, and other healthcare professionals.
  
  
• Communicate with patients and families in Polish when appropriate.
  
  
• Coordinate visit schedules directly with assigned patients.
  
  
• Complete all visit documentation within 48 business hours of the home visit.
  
  
• Maintain professional communication with DirectMed's clinical and administrative teams.
