Home Health Physical Therapist

Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

JOB DESCRIPTION


Active Health Services is seeking a Physical Therapist (PT) to cover the Southern west part of Riverside. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.


DUTIES AND RESPONSIBILITIES

• Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care
• Evaluate home environment
• Assess for muscle strength, mobility, gait, ROM—potential for rehab
• Direct physical therapy treatment
• Instruct patients and families/caregivers in the use and care of therapeutic appliances
• Determine priority needs for physical therapy
• Report to physician patient’s reaction to treatment or changes in condition
• Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient
• Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences
• Train patient in the use of prosthetic device
• Identify patient and family/caregiver needs for other home health services
• Prepare and submit clinical and progress summaries based on the attainment of goals
• Participate in discharge planning for patient
• Prepare and submit a clinical progress summary based on the attainment of goals
• Provide physical therapy consultation to home families/caregivers
• Provide in-service education programs for nursing organization personnel
• Participate in peer consultation process
• Supervise Physical Therapy Assistants according to organization policy and state regulations

JOB REQUIREMENTS

• CA Physical Therapy License
• Two (2)+ Years of physical therapy experience, home health experience preferred
• Demonstrate good verbal and written communication and organization skills
• Possess and maintain current CPR certification
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
