One-on-One Patient Care
Spend your time helping patients improve their strength, mobility, safety, and independence in the comfort of their own homes.
What You'll Do
Perform comprehensive physical therapy evaluations in patients' homes
Develop individualized plans of care based on patient needs and goals
Provide therapeutic exercises, mobility training, balance training, and other skilled PT interventions
Help patients improve strength, function, safety, and independence
Educate patients and caregivers on exercises, mobility techniques, and fall prevention
Monitor progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team
Complete accurate and timely clinical documentation
What You'll Bring
Degree from an accredited Physical Therapy program
Active Arizona Physical Therapist license
Current CPR/BLS certification
Home health experience preferred
Strong communication and organizational skills
Ability to work independently and manage a field-based schedule
Reliable transportation and valid driver's license
Flexibility to travel outside your primary territory when needed to maintain a full-time caseload
