Location: Phoenix, AZ – West Valley (territory west of I-17; formal map pending)
Employment Type: Direct Hire / Permanent
Compensation: $140,000 annual base + overage bonus + $2,000 sign-on bonus

About the Opportunity

A growing home health agency serving the West Valley of Phoenix is seeking a licensed Physical Therapist to join their team on a permanent, direct-hire basis. This is an immediate need — the agency is actively interviewing and ready to move quickly for the right candidate.

Position Summary

The Home Health Physical Therapist will provide skilled, in-home physical therapy services to patients throughout the West Valley territory (area west of I-17), delivering individualized treatment plans that support patient recovery, mobility, and independence within the home setting.

Key Responsibilities

• Conduct comprehensive in-home evaluations and develop individualized treatment plans

• Provide skilled PT interventions in accordance with physician orders and plan of care

• Document visits, progress notes, and outcomes in compliance with home health regulatory and payer requirements (Medicare/Medicaid guidelines)

• Collaborate with interdisciplinary care team members (nursing, OT, SLP, case management) to coordinate patient care

• Educate patients and caregivers on home exercise programs, safety, and self-management strategies

• Maintain accurate, timely clinical documentation using agency EMR system

• Manage a home visit caseload within an assigned territory efficiently and independently

Requirements

• Current, unrestricted Arizona PT license (or eligibility to obtain)

• Home Health experience required

• Valid driver's license, reliable transportation, and willingness to travel within the West Valley territory

• Strong clinical, organizational, and communication skills

• Ability to work independently in a home-based care setting

Compensation & Benefits

• Base Salary: $140,000/year

• Overage Bonus: Available for productivity above target visit volume

• Sign-On Bonus: $2,000

• Full-time, permanent direct-hire position
