Job Summary
The Physical Therapist reports to the Director of Professional Services. This role is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice.
Essential Functions of Position
- Participates in the development of patient physical therapy plans of care.
- Makes visits for assessments and evaluations, communicating with the physician as needed.
- Implements the plan of care and documents accurately and timely to maintain patient medical records.
- Provides education and instruction to team members, patients, and their families.
- Participates in agency quality assessment performance improvement (QAPI) programs.
- Ensures the growth and profitability of the company through responsible resource use and community education.
- Participates in interdisciplinary team meetings to coordinate care, exchange information, problem solve, and receive support and education.
- Applies agency policies and procedures and complies with federal and state regulations.
- Provides skilled restorative treatments for pain reduction and functional maintenance, including various therapies and safety instructions.
- Conducts initial and ongoing comprehensive assessments of patient needs, including OASIS assessments.
- Evaluates outcomes of care.
- Directs and supervises Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides.
- Accepts direction from the Director of Professional Services, Patient Care Coordinator, or Rehabilitation Supervisor.
Required Skills & Experience
- Knowledge of physical therapy principles, concepts, and methodology appropriate for terminally ill patients.
- Demonstrates problem-solving and decision-making abilities.
- Ability to manage patient care with specific experience in bedside nursing, symptom management, crisis intervention, and family intervention.
- Excellent communication skills.
- Ability to develop and maintain rapport with patients, families, and staff.
Preferred Skills & Experience
- One year or more work experience in physical therapy in a multidisciplinary setting is preferred.
Schedule Notes
Monday-Friday 8 a.m. - 5 p.m. No on-call duties. 60-mile travel radius. Estimated 6 patients per day.
