    Position Overview

    Home Health Physical Therapist Horizon Home Health & Hospice is seeking a full‑time, salaried Physical Therapist Case Manager to join our home‑health team in Caldwell, Idaho. This role is ideal for PTs who value autonomy, meaningful patient relationships, and the opportunity to deliver high‑quality care directly in patients’ homes.

    As a PT Case Manager, you will oversee a balanced caseload, provide skilled therapy interventions, coordinate interdisciplinary services, and guide patients through recovery, functional improvement, and safe transitions of care.

    Key Responsibilities

    PT Case Management Responsibilities

    • Perform comprehensive physical therapy evaluations in patient homes.

    • Develop individualized treatment plans focused on functional mobility, safety, and independence.

    • Provide therapeutic interventions including gait training, strengthening, balance work, and home‑exercise program development.

    • Conduct home‑safety assessments and recommend appropriate DME or environmental modifications.

    • Manage a field‑based caseload with strong organizational efficiency.

    • Coordinate care with interdisciplinary team members to ensure seamless service delivery.

    • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

    • Communicate effectively with physicians, caregivers, and internal team members.

    • Support safe transitions from hospitals, SNFs, and outpatient settings.

    Schedule

    Full‑Time Schedule

    • Full‑time, Monday–Friday

    • Normal business hours (daytime schedule)

    • Predictable workflow with full team availability during working hours

    Qualifications

    PT Requirements

    • Current Idaho Physical Therapist license (or ability to obtain).

    • One year of PT experience preferred; home‑health experience a plus.

    • Strong communication, assessment, and clinical reasoning skills.

    • Ability to work independently in a field‑based environment.

    • Reliable transportation and valid driver’s license.
