Rapidly growing agency with strong leadership Clinician-focused and supportive culture Flexible scheduling and autonomy Opportunity to grow with a fast-expanding organization Meaningful one-on-one patient care in the home settingKey ResponsibilitiesPatient Assessment & EvaluationConduct initial OASIS evaluations and in-home assessments to determine physical therapy needsReview patient medical histories, diagnoses, and physician ordersIdentify patient-specific goals and develop individualized treatment plansTreatment Planning & ImplementationDevelop and implement treatment plans to restore mobility, strength, and functionalityUtilize various therapy techniques including therapeutic exercise, manual therapy, and adaptive equipmentProvide cost-effective treatments that improve motion, mobility, and independenceReduce reliance on medications and provide non-surgical treatment alternatives when appropriatePatient EducationEducate patients and caregivers on exercises, recovery techniques, and safety measuresProvide guidance on proper body mechanics and injury preventionDevelop home exercise and wellness programs tailored to each patient's needsProgress MonitoringMonitor patient progress and adjust treatment plans as necessaryCommunicate regularly with physicians and healthcare team members regarding patient progressCollaborationWork closely with nurses, occupational therapists, physicians, and other healthcare professionalsParticipate in interdisciplinary care coordination to ensure comprehensive patient careDocumentation & ComplianceMaintain accurate and timely patient documentationEnsure compliance with HIPAA regulations and agency policiesComplete all documentation required for billing and insurance purposesQualificationsActive Arizona Physical Therapist (PT) LicenseHome Health experience preferredStrong clinical assessment and communication skillsAbility to work independently in a field-based environmentPassion for delivering patient-centered careTravelThis position requires travel to patient homes throughout assigned territories and the ability to manage a mobile caseload.
