What You’ll Do
Evaluate patients’ functional abilities in their home environment
Develop and implement individualized treatment plans
Help patients improve daily living skills and independence
Educate patients and families for long-term success
Collaborate with nurses, PTs, and interdisciplinary team members
Complete timely and accurate documentation
What You’ll Bring
Master’s degree in Occupational Therapy
Active Arizona OT license
Current CPR/BLS certification
Home health experience preferred
Strong communication and organizational skills
Reliable transportation and valid Arizona driver’s license
