Your Impact: Lead a compassionate home healthcare team dedicated to helping seniors thrive in their own homes. You'll oversee skilled nursing, therapy services, and care coordination ensuring high-quality, personalized support for our participants.
What You'll Do:
- Manage and mentor a team of home health nurses and therapists, fostering excellence in care delivery.
- Ensure all home health services align with care plans and regulatory standards.
- Oversee care quality in homes, nursing facilities, and board & care settings.
- Optimize workflows and policies to enhance efficiency and participant outcomes.
- Collaborate with interdisciplinary teams to address complex clinical needs.
What We're Looking For:
- Active California RN License (required).
- 2+ years of home health experience, preferably with seniors or frail populations.
- Strong clinical judgment and leadership skills.
- Passion for improving care for vulnerable seniors.
