Your Impact: Lead a compassionate home healthcare team dedicated to helping seniors thrive in their own homes. You'll oversee skilled nursing, therapy services, and care coordination ensuring high-quality, personalized support for our participants. What You'll Do:
Manage and mentor a team of home health nurses and therapists, fostering excellence in care delivery.
Ensure all home health services align with care plans and regulatory standards.
Oversee care quality in homes, nursing facilities, and board & care settings.
Optimize workflows and policies to enhance efficiency and participant outcomes.
Collaborate with interdisciplinary teams to address complex clinical needs.
What We're Looking For:
Active California RN License (required).
2+ years of home health experience, preferably with seniors or frail populations.
Strong clinical judgment and leadership skills.
Passion for improving care for vulnerable seniors.
