Heralds Home Health is expanding our nursing team and looking for experienced, reliable LVNs to provide skilled nursing visits throughout the Harbor Area.

- Wilmington
- Carson
Position: Per Diem / Field LVN
Responsibilities:
- Provide skilled nursing visits in patients' homes
- Monitor patient progress and report changes in condition
- Perform wound care and prescribed treatments
- Educate patients and caregivers
- Coordinate care with the RN Case Manager
- Complete visit documentation accurately and on time
Requirements:
- Current California LVN license
- Strong nursing assessment and clinical skills
- Home health experience preferred
- Reliable transportation
- Excellent communication and time-management skills
