Position: Per Diem Home Health LVN
Your Responsibilities:
- Conduct scheduled skilled nursing visits
- Monitor vital signs and patient conditions
- Provide wound care and ordered treatments
- Assist with medication management and patient education
- Communicate patient updates to the supervising RN
- Maintain timely and accurate clinical documentation
Qualifications:
- Active California LVN license
- Home health experience preferred
- Strong clinical skills
- Dependable transportation
- Excellent organizational skills
- Ability to work independently in the field
