Pay: $1,600/week
Schedule: Monday – Friday
Job Summary:
Seeking an experienced Home Health LPN to provide skilled nursing care to patients in their homes across the Fairfax and Prince William County area.
Responsibilities:
Deliver direct patient care in a home setting
Monitor patient conditions and report changes
Assist with care plans and patient education
Complete documentation in HCHB (Homecare Homebase)
Requirements:
Active LPN license (VA)
Minimum 1 year of home health experience
HCHB experience required
Valid driver’s license and reliable transportation
