Position Overview

We are seeking a dedicated Travel Licensed Practical Nurse (LPN) to provide high-quality skilled nursing care to patients in their homes. This role requires daily travel and a strong commitment to patient safety, clinical excellence, and compassionate care in a home health setting.

Key Responsibilities

The Travel LPN will perform skilled home health visits in accordance with home health guidelines, standards of practice, and physician orders. Daily responsibilities include, but are not limited to:

• Delivering safe, effective skilled nursing care in patient homes
• Performing comprehensive patient assessments and monitoring condition changes
• Providing wound care, including wound vac management and compression dressings
• Administering medications and treatments as prescribed
• Performing venipuncture when required
• Educating patients and caregivers on disease processes, medications, and care plans
• Accurately documenting all patient care and visits in the electronic medical record
• Communicating effectively with interdisciplinary care team members
• Adhering to all regulatory, safety, and infection control standards

Required Qualifications & Experience

• Active and valid LPN license (state-appropriate)
• CPR certification
• Ability to travel daily to patient homes
• Strong clinical judgment and attention to detail

Preferred Skills & Attributes

The following skills are not required but will set candidates apart:

• Experience with wound vacs
• Compression dressing application
• Homecare Homebase (HCHB) experience
• Venipuncture skills
• Strong time management and organizational abilities
• Excellent communication and patient education skills

Schedule & Work Environment

• Schedule: Monday–Friday
• Shift Hours: 8:00 AM – 4:30 PM
• Work Setting: Patient homes (home health)
• Travel: Required daily; may visit multiple patient locations per shift
