We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the Johnstown, PAarea. This position is eligible for $25,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $15,000 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!
What You’ll Do:
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
- Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
- Maintain accurate, timely, and compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

LPN Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience and the clinical expertise to deliver quality patient care
- Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
