One of our top clients, a respected home health agency in Federal Way, WA, is seeking a compassionate Licensed Practical Nurse to join its field team. The ideal candidate wants meaningful one-on-one patient care and a steady schedule. This is a full-time, Monday through Friday day-shift role with no nights or weekends.

Responsibilities:

• Provide skilled nursing visits to patients in their homes under the direction of an RN and the plan of care.
• Give medications, wound care, and treatments, and monitor vital signs and patient progress.
• Educate patients and families on care, safety, and disease management.
• Document visits accurately and on time.
• Communicate changes in condition to the RN case manager and physicians.

Qualifications:

• Active Washington State LPN license
• Home health experience preferred; strong LPNs from other settings are welcome to apply
• Current CPR/BLS certification
• Valid driver's license, reliable transportation, and auto insurance
• Strong communication and time management skills
• Ability to work independently and as part of a team.
