What You'll Do
- Provide skilled nursing care in patients' homes
- Deliver individualized care based on established care plans
- Monitor patient condition and report changes to supervising clinical staff
- Educate patients and families on basic health maintenance and care needs
- Maintain accurate documentation and charting
- Collaborate with the BrightStar Care team to ensure quality patient care
Qualifications
- Active LVN license
- Home health, private duty, hospice, skilled nursing, or acute care experience preferred
- Strong clinical, organizational, and communication skills
- Ability to work independently and manage time effectively
- Reliable transportation and willingness to travel within assigned service area
- A commitment to compassionate, high-quality patient care
