    Job Summary
    We are seeking a Home Health Licensed Practical Nurse (LPN) to join our team. In this role, you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You are a compassionate and detail-oriented professional who takes pride in delivering personalized care, observing patient conditions, and reporting changes accurately. To succeed, you should have a solid foundation in nursing practices and a commitment to patient-centered care.

    Responsibilities
    • Provide direct nursing care to patients in accordance with the established plan of car
    • Assist in evaluating patient needs and communicate findings to the supervising RN
    • Administer medications and treatments as prescribed, and monitor for side effects or adverse reactions
    • Document all patient care activities and changes in condition promptly and accurately
    • Maintain communication with the interdisciplinary team and follow up on care instructions
    • Educate patients and caregivers on basic health and personal care strategies
    • Adhere to infection control and safety protocols in the home setting
    Qualifications
    • Currently licensed Practical Nurse (LPN) in the state of Florida
    • Previous experience in home health or similar setting is preferred
    • Strong clinical and communication skills
    • Valid driver’s license and reliable transportation
    • Current CPR/First Aid certification required
