Home Health Coordinator RN

Position: Home Health Intake Coordinator RN

Shift: 8:30a-4:30p, 1 weekend per month

Salary: $35 -$50 p/hr

Position Information: As an RN Home Care Intake Coordinator, you will be responsible for assessing patients home health care needs, coordinating referrals to home care and hospice, and ensuring the best patient outcomes. You will play a vital role in providing compassionate care and promoting continuity of care for our client. This position is located in New Brunswick, NJ.

Qualifications:

• Associate degree or higher
• Community Health (2+ years)
• Hoome Health / Hospice exp a plus!
• Valid Registered Nurse (RN) in the state of New Jersey (NJ)
• Valid American Heart Association Basic Life Support (AHA BLS)
