Responsibilities:
What You Will Do:
Organize, develop, manage, and direct the home health functions of your community health hub
Manage a team of clinicians to ensure our patients receive exceptional care in their homes
Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
Conduct patient visits (as needed)
Manage emergency situations in a prompt, precise, and professional manner
Complete medication administration and treatment needs for all assigned patients
Provide nursing care in accordance with applicable regulations and Company policy
Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Qualifications:
Qualifications:
Active RN state license
Two or more years of nursing experience (supervisory experience preferred)
Two or more years of home health nursing experience required
Valid drivers’ license
