Schedule: Friday–Monday | Weekend on-call rotation
Productivity: 30 points/week
License: Indiana RN license or Compact license accepted
Mileage: $0.43/mile
Coverage: Clark, Floyd, Scott, Jefferson, Crawford, Harrison, Washington & portions of Orange County, IN
We’re seeking an experienced Home Health RN for a Baylor schedule serving patients throughout Southern Indiana. This role is ideal for an RN with strong clinical skills and experience managing complex home-care needs.
Key Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Provide skilled home health services and coordinate care with physicians and other healthcare professionals.
- Manage care transitions and connect patients with appropriate medical and community resources.
- Monitor patient progress and identify opportunities for prevention and improved outcomes.
- Maintain accurate clinical documentation and advocate for patient needs.
Required Skills
- Home Health experience required
- HCHB experience required
- Wound care
- IV therapy
- Drain care
- G-tube care
- Patient education and basic health teaching
Qualifications
- Associate’s or Bachelor’s degree in Nursing or related field.
- Current, unrestricted Indiana RN license or Compact license.
- Case Management certification.
- Strong clinical assessment, critical-thinking, communication, and care-coordination skills.
