About the Role:
We are looking for a dedicated and detail-oriented Home Health Care Intake Coordinator to join our growing team in Glendale, CA. In this vital role, you'll be the first point of contact for patients and families, helping connect them with the compassionate care they need. Responsibilities:

• Manage the end-to-end patient intake process, including referral intake, verification, and coordination of care
• Communicate with physicians, hospitals, and referral sources to obtain necessary documentation and authorizations
• Verify patient insurance eligibility and obtain prior authorizations for home health services
• Coordinate with clinical staff to ensure timely and appropriate patient admissions
• Maintain accurate and up-to-date patient records in the EMR/EHR system
• Serve as a liaison between patients, families, referral sources, and internal care teams
• Ensure compliance with Medicare, Medi-Cal, and private insurance guidelines during the intake process

Requirements:

• Minimum 1-2 years of experience in home health, medical office, or healthcare intake coordination
• Familiarity with Medicare, Medi-Cal, and private insurance authorization processes
• Strong knowledge of home health regulations and compliance standards
• Proficiency with EMR/EHR systems and Microsoft Office Suite
• Excellent communication, organizational, and multitasking skills
• Compassionate, patient-focused demeanor with strong attention to detail
• Bilingual skills (English and Armenian) Required
