About the Role:
We are looking for a dedicated and compassionate Home Health Care Case Manager/Intake Coordinator to join our growing team in Glendale, CA. In this dynamic dual role, you'll be the vital link between patients, families, and care teams — ensuring seamless admissions and high-quality, coordinated home health services. Responsibilities:
  • Manage the end-to-end intake process for new home health patients, including referral intake, eligibility verification, and authorization
  • Coordinate and oversee individualized patient care plans in collaboration with physicians, nurses, and therapists
  • Serve as the primary point of contact for patients, families, referral sources, and insurance providers
  • Monitor patient progress and ensure compliance with care plan goals and regulatory requirements
  • Facilitate smooth transitions of care from hospital, SNF, or clinic settings to home health services
  • Maintain accurate and timely documentation in the EMR/EHR system
  • Communicate with payers regarding authorizations, denials, and appeals as needed
Requirements:
  • Active RN, LVN, or relevant clinical license preferred; case management or healthcare coordination experience required
  • Prior experience in home health, intake coordination, or case management strongly preferred
  • Knowledge of Medicare, Medi-Cal, and private insurance home health guidelines
  • Strong organizational and multitasking skills in a fast-paced environment
  • Excellent communication and interpersonal skills with patients, families, and referral partners
  • Proficiency with EMR/EHR systems and standard office software
  • Bilingual skills (English and Armenian) a plus in the Glendale community
