Responsibilities
- Assist patients with activities of daily living including bathing, dressing, grooming, toileting, and mobility support
- Provide companionship and emotional support to promote patient well-being
- Monitor patient condition and report changes or concerns to nursing staff or supervisors
- Assist with medication reminders and basic household tasks as needed
- Maintain accurate records of care provided and communicate effectively with healthcare team members and family caregivers
- Follow individualized care plans and instructions carefully
- Adhere to all safety, confidentiality, and infection control procedures
Qualifications
- High school diploma or equivalent required
- Previous Home Health Aide or caregiving experience preferred
- Home Health Aide (HHA) certification preferred or willingness to obtain certification in accordance with New Hampshire regulations
- Strong interpersonal and communication skills
- Compassionate, dependable, and patient-focused attitude
- Ability to pass background check and drug screening
- Reliable transportation preferred
- Ability to work independently while collaborating with a healthcare team
