What You'll Do
As an HHA or CNA with Elite Home Healthcare, you'll provide individualized care and support to patients in their homes according to the patient's Plan of Care and under appropriate clinical direction and supervision.
Depending on the patient's Plan of Care, your qualifications, demonstrated competency, and assigned responsibilities, duties may include:
- Assisting with activities of daily living (ADLs), including bathing, grooming, dressing, toileting and personal hygiene
- Providing personal care and assistance with mobility, transfers and positioning
- Performing assigned care tasks within your demonstrated competency and authorized responsibilities
- Providing household services essential to the patient's health and safety
- Assisting with medications that are ordinarily self-administered when permitted by applicable requirements, the patient's Plan of Care and Elite policy
- Observing patients for changes in condition, behavior or needs and promptly reporting concerns to the supervising Registered Nurse
- Following each patient's individualized Plan of Care
- Accurately documenting services provided, care completed and relevant observations
- Completing required visit documentation according to Agency requirements
- Following infection-control, patient-safety and bloodborne-pathogen procedures
- Maintaining patient privacy and confidentiality
- Communicating effectively with patients, families, nurses, supervisors and other members of the care team
- Participating in required training and continuing education
HHA / CNA Qualifications
Candidates must:
- Be at least 18 years of age
- Meet applicable Indiana requirements for the position and services being provided
- Maintain required certification and/or registry status, as applicable
- Complete required competency evaluations and demonstrate competency for assigned patient-care tasks
- Be able to read, understand and follow a written Plan of Care
- Have strong verbal, written and interpersonal communication skills
- Be dependable and committed to accepting scheduled patient care once an assignment is accepted
- Complete all applicable Elite Home Healthcare employment and compliance requirements before providing patient care

Work Environment & Physical Requirements
Care is primarily provided in patients' homes.
The position may involve exposure to communicable diseases, bloodborne pathogens and other potentially infectious materials. Employees are required to follow applicable infection-control and safety procedures and use appropriate protective equipment when required.
The position requires the physical ability to perform assigned patient-care responsibilities, including occasional lifting, pushing or pulling of up to 50 pounds and safely assisting patients with activities identified in their Plan of Care.
