In this key administrative role, you will coordinate, track, and optimize scheduling assignments for Home Care Assistants and field staff. Working under the guidance of the Home Care Supervisor, you will ensure our patients receive timely, high-quality care at home while managing schedule adjustments, processing EMR authorizations, and verifying care logs. Serving as a crucial liaison between patients, families, field staff, and vendors, you will deliver exceptional customer service and keep daily operations running seamlessly. Effectively coordinate the scheduling of home care personal care assignments as directed by the Home Care Supervisor, including adjusting schedules based on changes and communicating promptly with all stakeholders (patients, families, home services staff, etc.).
Ensure required authorizations are in place in the Electronic Medical Record (EMR) for approved personal care services.
Ensure all Care Logs and Timesheets are received from vendors and in-home staff visits in a timely manner.
Consistently implement procedures to ensure a successful experience for providers, staff, and patients, such as making reminder calls, offering instructions, or sending materials ahead of time.
Maintain current and accurate records of all scheduled home care personal care assignments, including scheduled times and visit outcomes (e.g., visit occurred or did not occur, reasons for rescheduling).
Complete all required documentation in a timely and accurate manner.
Protect privacy and maintain strict confidentiality of all company procedures, patient data, and family information.
Follow company policies and procedures and participate in Quality Improvement activities, staff training, and team meetings.
Communicate regularly with the Supervisor, Home Care Coordinators, and Home Services staff regarding workload and priorities.
Complete all mandated trainings, education, and occupational health screenings in a timely manner.
Exercise flexibility in performing assignments as business needs evolve, including the willingness and ability to work a varied schedule that may include evenings, weekends, and overtime if needed.
Perform other related duties as assigned.
High School Diploma or GED equivalent.
Minimum of one (1) year of experience as a scheduler within a home health or home care agency setting.
Ability to process accurate information in a timely, efficient manner.
Excellent organizational, communication, and interpersonal skills.
Demonstrated ability to prioritize tasks, follow directions, and manage demands or staff schedules in a fast-paced environment.
Ability to work independently with minimal supervision.
Experience and competency in working respectfully with individuals from diverse backgrounds and cultures.

Minimum of one (1) year of hands-on experience working with Electronic Medical Record (EMR) systems and dedicated scheduling software applications.
Work Type: Full-Time Contract Position (40 hours per week).
Work Setting: On-Site Center / Office Environment located in San Jose, CA.
Schedule:Monday through Friday, 8:00 AM – 4:30 PM (40 hours per week, with flexibility required for varied shifts if business needs evolve).
