Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health RN Case Manager.
Coverage Territory to include in and around the
Avon, Braintree, Canton, Holbrook, Mattapan, Milton, Quincy, Randolph, Stoughton, MA area.
- Monday-Friday, 40 hours/week
- Full-time, Contract
- Every 6th weekend rotation
- Highly competitive pay rates with IRS mileage reimbursement
The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare Certified agency. Great benefits include:- Health, Dental, Vision
