Responsibilities

Build relationships with healthcare professionals
Visit facilities and introduce our services
Develop referral partnerships and follow up
Track outreach and referrals

Requirements

5+ years of experience in social work, discharge planning, or a related field
Knowledge of Omaha’s healthcare or senior care community preferred
Strong communication and networking skills
Self-motivated with reliable transportation
